Services are provided to eligible persons of any age (birth thru old age).
Eligibility:
Individuals must have a diagnosed developmental disability that started
before age 18 for mental retardation or before age 22 for other developmental
disabilities, and is expected to last indefinitely. Individuals must have
significant impairment in adaptive functioning not otherwise attributed to
mental illness, substance abuse, emotional disorder, Attention Deficit and
Hyperactive Disorder, a learning disability or sensory impairment.
Document of disability:
For children age 5 and younger: a statement from a medical practitioner
that the child has a condition or syndrome that will likely cause significant
adaptive impairment.
For children age 6 and older or adults: a diagnosis of mental
retardation or of a developmental disability and significant adaptive
impairments.
Eligibility for children ages birth to 17 is always provisional with
reassessment at age 6 and before age 18 for mental retardation or age 22 for
other developmental disabilities.
