Summary of Program
In Oregon, we call this program
"Living Well", but it
is the same program developed by Stanford University called Chronic Disease
Self-Management Program (CDSMP). If you would like more information on
CDSMP, how they did the research, outcomes, or how to set-up your own program,

The Lincoln County "Living Well With Chronic Conditions Program" consists of many collaborative partners who are concerned about not only the cost of inadequate care’s adverse impact on the health care system but also the adverse impact on the quality of life of individuals with, or at risk for chronic disease.
Are you living with a chronic health
condition?
You will get the support you need, find practical ways to deal with pain and
fatigue, discover better nutrition and exercise choices, and learn better ways
to talk with your doctor and family about your health.
If you have conditions such as diabetes, arthritis, high blood pressure,
heart disease, chronic pain or anxiety; the Living Well Workshop can help you
take charge of your life.
The program is striving to work towards the following objectives:

Partnering with local healthcare providers and concerned
organizations who deal specifically with individuals who have chronic
conditions; to promote the CDSMP and Living Well With Chronic Conditions.

Seek sustainable resources to support the on-going implementation of
cost-effective interventions and changes.

 |
Seek funding to help build infrastructure. |
 |
Promote the importance of self-management and treatment for
people living with a chronic condition.

 |
Increase the size and skill of the current Living Well Leader
Team.
|
 |
Provide a minimum of 12 yearly Workshops throughout the county;
making these sessions available to all residents in their own communities.
|
This program is unavailable to provide direct services.

Join a free 2
1/2 hour Living Well Workshop, held each
week for six weeks. Classes are fun and interactive.
|
 |
Learn from trained volunteer leaders with
health conditions themselves,
about how to manage symptoms and medication, work with your healthcare team,
set weekly goals, problem-solve effectively, improve communications, relax,
handle difficult emotions, ear well and exercise safely.
