Q: What is the Lincoln Community Health Center?
A: Lincoln Community Health Center is a collaboration of community organizations, county government, and other committed partners who joined together to provide access to affordable health care for the citizens of Lincoln County.

Q: Who cares for you?
A: The providers at Lincoln Community Health Center are Physicians, Nurse Practitioners and Registered Nurses prepared to manage both chronic and acute health care needs of all age groups, from infants to older adults.


Q: What can the providers do?
A: Our providers have skills in identification, screening and triage of acute episodic illnesses and nursing and medical management of commonly encountered acute and chronic illnesses.


Q: With whom do the providers collaborate?
A: Our providers work and consult with other providers as necessary and are developing a knowledge base of community needs and resources available for health promotion, health education, guidance and counseling for patients and their families concerning illness; its prevention, health promotion, maintenance, and management.
